Job Summary

Provides leadership and management for assigned organ transplant programs. Responsible for the nursing component of the following: management of patient care delivery, human resource management, performance improvement, fiscal management, management of organizational change and operations related to future clinical development and success of the program. The manager will demonstrate a commitment to staff education and development.

Essential Functions

Must perform all duties and responsibilities in accordance with the Service Standards of the Hospital(s).

Management of Patient Care Delivery: Assures the delivery of quality, therapeutic and cost-effective care to transplant patients in the inpatient and outpatient settings. Demonstrates the ability to consider age-appropriate needs when interacting with and caring for neonates, infants, pediatric, and adolescent patients.

• Assures that care given to patients is in accordance with standards of practice and desired patient outcomes.

• Determines an appropriate delivery of care model for the Service in cooperation with Patient Care Management, unit staff and physicians. Implements and evaluates safe, therapeutic, cost-effective innovative patient care practices.

• Assures efficient, effective work flow to meet the needs of patients and staff, including adequate number of staff, materials and supplies.

• Assures development, maintenance, accurate interpretation and use of policies and procedures on the unit and within the organization.

• Assures Program¿s compliance with the state, federal and regulatory mandates (Joint Commission, Title 22, CCS, UNOS, CMS, and California Nurse Practice Act). Develops action plans to ensure on-going compliance with all regulatory mandates. Submits UNOS data and quarterly Medicare cost report surveys on a timely basis.

• Manages work flow within given fiscal parameters. When patient care requirements or other operational needs obligate use of resources beyond monthly portion of annual resource allotment, provides justification to the PCD and implements agreed-upon plan.

• Demonstrates an ability to recognize management problems and collaboratively develops solutions.

• Demonstrates appropriate judgment in referring unresolved and recurrent issues or problems.

• Assists in the provision of patient care when necessary; maintains clinical competence.

• Identifies and/or responds effectively to the services changing needs and helps with priority setting. Anticipates potential problems and formulates plans to prevent them.

• Promotes policies and practices that treat family members as valued members of the health care team.

• Works with team on developing and implementing patient teaching guidelines and checklists.

• Meets regularly with the managers of units that interact with the transplant populations. Ensures compliance with hospital policies and procedures and HIPPA.

Human Resource Management: Communicates staff and management performance and expectations. Manages human resources effectively.  Provides ongoing and annual performance feedback and evaluation.

• Works with Patient Care Services and Human Resources to attract, recruit and retain qualified applicants.

• Interviews and staff in a timely manner according to appropriate hospital, regulatory and federal guidelines.

• Develops and maintains an adequate position control to provide
cost-effective, quality patient care services.

• Ensures that staff participates in competency-based orientation and training that is current, effective and outcome-oriented.

• Effectively coaches and counsels staff to promote a higher level of development.  Lends support and guidance as appropriate.

• Assists staff to develop and implement measurable professional goals reflecting the hospital's mission and strategic imperatives as well as individual learning needs.

• Works collaboratively with other departments to meet unit and organizational needs.

• Resolves conflicts constructively using appropriate management strategies.

• Effectively uses the corrective action process when necessary.

• Evaluates ways to use technology to improve efficiency and effectiveness of care.

Performance Improvement (PI): Strives toward high performance and improvement in all operations. Develops, implements, and evaluates PI projects/processes in order to continually monitor and improve patient care.

• Clearly identifies opportunities to improve quality, timeliness, efficiency and effectiveness of transplant and services.

• Collaborates in the development of processes that monitor patient care delivery.

• Uses a variety of analytical techniques to solve problems.

• Evaluates and assesses outcomes related to PI processes.

• Communicates processes and outcomes to all levels of staff and other disciplines.

• Orients and educates staff to their role in contributing to the hospital wide and unit based performance improvement processes.

• Develops and implements a written quality improvement plan which is evaluated and updated at a minimum annually.

• Works with management to complete the transplant dashboard.

• Works with Transplant Donor Networks to monitor accuracy and completion of donor activity and data and to ensure quality of on-call operations. Provides data as needed.

Fiscal/Revenue Management: Manages unit workflow and patient care delivery with the goal of achieving optimum financial position.

• Develops a patient care financial management plan for the Service including operational and capital budgets.

• Communicates fiscal expectations for the Service (updates on unit performance, variances and necessary fiscal adjustments) to physicians and staff.

• Effectively controls costs through economical utilization of personnel, materials and equipment, taking into consideration patient quality issues.

• Collaborates with case managers, physicians and other disciplines to assure a cost-effective approach to individual patient care from admission through discharge.

• Creates and monitors budget statements and financial reports (excluding academic and research budgets and reports) for the Services. Prepares variance reports and implements corrective action to maintain service operations within budget constraints.

• Solicits input and feedback, and develops action plans on opportunities or methods to improve service cost-effectiveness and enhance revenue.

• Maximizes revenue by tracking all living related donor charges, cadaveric charges, travel and surgical fees related to organ acquisition.  Verifies data for the monthly statistics in renal transplant.

• Works with staff and other departments in providing clinical information for contracting proposals and negotiations. Responds on a timely basis to requests for information from the insurers and organ procurement agencies.

• Monitors and utilizes resources to ensure that all Transplant billing is in compliance with governmental agencies (Medicare, MediCal etc.).

• Coordinates the implementation of CCS billing in the Transplant, clinical areas. Provides ongoing monitoring of compliance in this area. Reviews fiscal implications on a regular basis.

Operations:


• Consistent with duties identified above, assumes 24 hour a day accountability for assigned nursing departments(transplant).

• Assists Patient Care Director and physicians in achieving and maintaining a viable cost structure while delivering "best practice" quality and clinical services.

• Incorporates continuous performance improvement perspective into ongoing operations.

• Coordinates and integrates service intra-and inter-departmentally to ensure coordination of patient care.

• Provides appropriate orientation, education and on-going staff development to assure the provision of quality patient care. Assure appropriate number of qualified and competent staff to provide appropriate patient care services.

• Manages patient care according to unit-specific written goals and scope of service plan.

• Assures the delivery of quality, therapeutic and cost-effective care to patients and families.

• Collaborates with business development on initiatives to increase program growth, analyze and improve patient satisfaction and other initiatives.

• Collaborates with PR, marketing and physicians in the production of the programs annual report.

• Reviews website material annually and updates site as appropriate.

• Other duties as assigned.

Minimum Qualifications

Any combination of education and experience that would likely provide the required knowledge, skills and abilities as well as possession of any required licenses or certifications is qualifying.
Education: Bachelor of Science in Nursing or a related health /business field preferred

Experience: Three to five years of experience as a  transplant coordinator preferred. Pediatric experience preferred

License/Certification: PA - Valid California license to practice as a Physician Assistant - OR - RN - Valid California license to practice as a Registered Nurse or NP - Valid California license to practice as a Nurse Practitioner

Pay Range

Compensation is based on the level and requirements of the role.

Salary within our ranges may also be determined by your education, experience, knowledge, skills, location, and abilities, as required by the role, as well as internal equity and alignment with market data.

Typically, new team members join at the minimum to mid salary range.

Minimum to Midpoint Range (1.0 FTE): $178,984.00 to $220,656.80
